Fill out the new patient form and send it to us via our contact form or email it to us to office@mylgroupservices.com to save time when you come in for your appointment.
What to Expect After you’ve scheduled your Telehealth appointment, check your email for a confirmation. If you don’t see it in your inbox, check your Spam or Junk folder. Still don’t see it? Call us back at (669) 235-4347 to have us re-send your invite.
Arriving for Your Appointment (Virtually, of course)
We ask that you click on the link on the email that we sent you, and check in 5-10 minutes before your appointment. Then, just accept the Terms & Conditions and you’ll be ready to start as soon as your doctor or other medical staff arrives. If you’re using your phone or tablet, make sure it’s charged at least 50%. We’ll see you soon.
